Let's talk about Urinary Incontinence

Urinary Incontinence (UI) means: Any involuntary or unwanted loss or leakage of urine. According to the World Health Organization (WHO), Urinary Incontinence is a widespread global disease and one of the medical taboos for many people. UI affects around one in three women worldwide.

Urinary Incontinence affects 17-45% of women worldwide and Stress Urinary Incontinence (SUI) is responsible for 48% of all cases.

It is common for other symptoms to occur along with Urinary Incontinence - having a strong urge to urinate, urinating more often than what is usual, waking from sleep to urinate, painful urination, urine leakage while asleep.

Urinary Incontinence in women can be divided into three main types:

1. Stress Urinary Incontinence (SUI) is urine leakage while coughing, laughing, or sneezing. Leaks can also happen when a woman walks, runs, or exercises. SUI is considered a social stigma. SUI can have a profound impact on women's quality of life as the condition can result in embarrassment, psychological distress, social isolation and loneliness. Only 27% of women with SUI seek help, as they perceive this problem as an occurrence of natural aging process. Factors that increase a woman's risk of developing SUI, or make the condition worse: genetic & hormonal factors, pregnancy, lifestyle factors e.g. aging, obesity, lung disease and smoking (leading to increased coughing), urinary tract infections, neurological diseases, pelvic floor disorders such as vaginal or uterine prolapse, neurological and muscular abnormalities.

2. Urge Urinary Incontinence (UUI) is a sudden strong urge to urinate that is hard to stop. Women with this type of urinary incontinence may leak urine on way to the bathroom too.

3. Mixed incontinence combines symptoms of both SUI and Urge Urinary Incontinence (UUI).

Treatment options for Urinary Incontinence (UI)

Life style modification, kegel's exercises are preventive measures. For Urge Incontinence (UUI) treatment of choice is medical management where as in SUI medicines are not effective. In early cases of SUI (grade I & II) non surgical options -LASER is one of the most preferred choice. Studies in India and abroad have proved its role in SUI.

MY EXPERIENCE

I was invited for the launch & experiential review on the all new “Alma Lasers Accent Prime Body Sculpting Treatment” at the Banjara Hills facility of Dr. Sudha Vani’s Sri Skin & Cosmetology Centre.

I opted to go for “upper arms” treatment where I have sagging skin from weight loss.

I have undergone under arm laser hair removal, like 10 years back & knew what to expect. But still was slightly nervous, as I have no experience with body contouring treatments.

Once I was taken to the procedure room, I heard the radio playing and immediately relaxed – since it was not just going to be me, the aesthetician & the machine! The aestheticians were also very warm & friendly, we kept having casual chats to keep my mind off the “sculptor” at work & the heat its producing!

A quick consultation happened, where it was diagnosed that I do not have fat to break down in my upper arms, but only the skin needs to be tightened. So, only radio frequency was to be used in my treatment procedure.

Accent Prime uses ultrasound waves to break down fat cells under the skin and then radio frequency for skin tightening by stimulating the collagen production by the body in the given area.

The treatment started off with me lying on the bed with my arm held up in an upward stretch. Oil was applied on the area of treatment – whole of my upper arm & arm pits.

The Accent Prime hand piece used looked like a big massager head and the aesthetician moved it over my upper arm in rotating movements.

The treatment went on for 10 minutes for each arm, at the end of which my hands were sweaty and felt quite hot to touch. There were also times during the treatment when I felt the laser was a little too hot to bear and as I signal, the treatment was intermediately stopped and then continued once I felt better. Sometimes more oil was applied too, for smoother movement of the hand piece on the skin. The aestheticians kept checking my comfort level throughout the procedure.

Both before and after pictures were taken of my upper arm (that I am shy to share here!) and one could clearly see how much the sagging skin had tightened and my skin felt plump & healthy, not loose & dry.

I noticed moderate redness of the skin, which is normal post any laser treatment.

One sitting of Accent Prime can bring about 1-1.5 inch loss (varies for individuals) and the number of sittings required also depends on the individual’s body requirements.

I was prescribed 2 more sittings of Accent Prime to have my flabby arms fully tightened and in shape, that I no more need to be conscious to wear sleeveless clothes. I really look forward to it!

One is also advised to drink lots of water before and after the procedure, to help the body remove fat cells effectively and also boost collagen production.

WHERE DOES THE FAT GO?

Accent Prime targets subcutaneous fat – meaning fat deposit under the skin & over the muscle, which is resistant to diet & exercises. It can also target visceral fat, which is fat stored in the abdominal cavity – another stubborn fat that can be reduced with diet/exercise, but leaves flabby skin behind.

Ultrasound waves from Accent Prime target these fats and break down the fat cells. These are cleared through the lymphatic system – the body’s own drainage & fat clearance system!

HOW IS SKIN TIGHTENED?

The radio frequency used in Accent Prime can reach upto 25 mm deep in the skin and causes dermal heating. It causes rotation and friction in between water molecules present in the skin. The fibroblasts also get stimulated and hence there is a boost in collagen production, resulting ultimately in skin tightening.

Fibroblasts are tissues in the body that are responsible for collagen production. Collagen is an important factor in aging – less collagen means wrinkles, lines & sagging skin.
